Support Coordination
Daisy Care Plus provides comprehensive support coordination services to help clients navigate and access the full range of services they need. Our experienced support coordinators work closely with individuals and their families to develop personalized care plans, connect with relevant providers, and ensure that all necessary services are delivered effectively. We assist with arranging healthcare, community programs, and personal care services, while advocating for the client’s needs and preferences. Our goal is to empower clients by simplifying the coordination process, allowing them to focus on their well-being and achieve their goals with confidence and ease.
All our staff are fully vaccinated against Flu, and Covid and possess current practicing certificate (where applicable).
